Release of Liability – Please read before completing registration

By completing this online registration, I understand that the sport of basketball has some inherent risks and injury can occur. On rare occasions these injuries can be serious and in even more rare situations, can result in death. Should a medical need arise, I authorize the coach or designate to seek medical attention as required. In consideration of my child being allowed to participate in the Dauphin Hawks Basketball program, I, the parent/guardian assume all the risk and agree to hold the Dauphin Hawks Program, directors, officers, coaches and rented facilities harmless from any and all injuries, liabilities or claims.  

I also give permission for the Dauphin Hawks program to use my child’s name, photo or video for promotional purposes.
